Initial Comments:
An off-site follow up survey conducted on July 20, 2022, found that Adult Transitional Care, LLC, had corrected the deficiences and was found in compliance with the requirements of 28 PA Code, Part IV, Health Facilities, Subpart H, Chapter 611 Home Care Agencies and Home Care Registries. The deficiencies were cited as a result of an home care agency state relicensure survey conducted on May 18, 2022.
